The biophysics of vascular compliance: How loss of elastin fibers accelerates pulse wave reflection and increases cardiac ventricular work.

1. Pulse Wave Velocity (PWV) and Elasticity

Every contraction of the left ventricle ejects a volume pulse into the aorta, generating a pressure wave that propagates along the arterial tree. In young, compliant arteries, this pulse wave travels slowly. As vessel walls stiffen due to elastin degradation and collagen cross-linking, Pulse Wave Velocity (PWV) accelerates dramatically.

2. Wave Reflection and Ventricular Afterload

When the forward pressure wave hits peripheral arterial branching points, a reflected wave travels backward toward the heart. In stiffened arteries, the reflected wave returns prematurely during systole rather than diastole, augmenting systolic pressure and increasing cardiac afterload.
